Prior to the adoption of 8.04.030 (Ord. 2180) on 06/16/2003, Section 8.04.030 read as follows.

    In addition to the base fees provided for in Sections 8.04.010 and 8.04.020 of this chapter, each person treated and/or transported by the Columbia, Illinois ambulance service shall pay the following fee for the following products and/or services necessary to be used in connection with their treatment and services rendered by the Columbia, Illinois ambulance service:

Use of cardiac monitor    5.00
Use of cervical collar    15.00
Mileage assessment for
  ambulance transport    4.00 per mile

    The Columbia, Illinois ambulance service shall submit itemized bills to patients for whom services have been provided by the ambulance service which will describe the base fees and special service charges assessed. The mileage charge shall be a minimum of four dollars ($4.00) for each mile or fraction thereof traveled in transport of the patient (that is to say, the mileage assessment shall not be pro-rated for a fraction of the last mile of transport traveled).
